The Diabetes Educator is involved with assessment, diagnosis, educating and proactively manage the patient's condition. This position will be primarily at the South Family Medicine Clinic with every 7th weekend call rotation at the Medical Center.

  • Monday - Friday 8a-5p + 7th weekend Call

Job Summary

Educates patients on self-care involved in the diagnosis of diabetes; collects patient data to monitor health status and proactively manage the patient's condition. Organizes diabetes self-care materials including nutrition, medication, physical activity and glucose testing. Educates patients on the necessary lifestyle changes involved in the diagnosis of diabetes. Demonstrates familiarity with pathophysiology, epidemiology, and clinical practice consistent with practice level. Collects patient data on a regular basis, in order to monitor their health status. Makes suggestions on how to improve health conditions based on the analysis of patient data; refers patients with specific problems to other healthcare providers. Applies principles of program and/or business management to create a climate that supports successful self-management of diabetes. Educates patients on self-care and lifestyle changes involved in the diagnosis of diabetes.

Qualifications

Must have a bachelor’s degree in nursing or willing to obtain within three years. Graduate from an accredited nursing program preferred, including, but not limited to, American Association of Colleges of Nursing (AACN), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA) preferred.

Two years of experience as a clinical nurse is required with demonstrated leadership skills. Previous experience and knowledge of Diabetes Management preferred.

Requires current licensure as a Registered Nurse in state(s) of practice and/or possess multi-state licensure privileges as required by position. Nurses performing nursing practice over the telephone require licensure in the states where the patients being served reside. Must hold certification as a Certified Diabetes Educator from the National Certification Board of Diabetes Educators or obtain that certification within three years when eligible. Basic Life Support (BLS) certification is required.
