Job Summary

Displays ability to apply the knowledge and skills of advanced social work practice during the pre and post transplant process. Will assume a social worker role during the transplant process, providing psychosocial support and advocacy to patients and their families. The responsibilities include conducting assessments to evaluate the patient's mental and social readiness for transplant, counseling to patient and family members to help with the emotional challenges of transplant, providing thorough education throughout the transplant process, facilitating ongoing support and resources, and assist with the financial planning. They work as part of a multidisciplinary team, collaborating with doctors, nurses, dietician, pharmacist, and financial advocate to ensure the best possible outcomes for transplant patients.

Graduate from an accredited school, currently holds an unencumbered MSW license with the state where the practice is occurring: South Dakota, Minnesota and Iowa. Obtains and subsequently maintains required department specific competencies and certifications.

Qualifications

Master’s degree in Social Work from an accredited curriculum required.

In Minnesota, must follow Minnesota State statute regarding social worker education.

Healthcare and/or mental health hospital experience preferred.

Must possess a license in good standing in state(s) of practice:

In South Dakota, must have one of the following:

Certified Social Worker (CSW)

Certified Social Worker - Private Independent Practice (CSW-PIP)
